About the financial assessment
Applying for funding towards your care and support services
This form is used to determine if Hampshire County Council will contribute towards the cost of your care and support services. Please ensure you have read our financial assessment information and the linked pages about paying for care at home and paying for care in a care home, before completing this form. This will explain whether you are likely to be eligible for funding from us. If you would like a quick indicator of whether you may be eligible for funding from us, if you have eligible care needs, you can complete our financial checker.
This form should only be completed if you have been assessed as being eligible to receive care from the County Council.
Before you complete this form, please ensure that you have read the ‘Your Information’ booklet which explains how we process and share your information.
How long will it take?
The form should take around 20 to 40 minutes to complete. Allow extra time for gathering evidence of your finances, which you will need to send us. See Providing evidence for more information about the type of documents you will need to provide.
There is the option to save your progress at any point once you have started the form, so you can come back to it later.
What happens next?
You will be informed in writing of the outcome of your financial assessment (even when the assessed contribution is nil) and how to pay the amounts due.
Completing this form does not mean that Hampshire County Council will contribute towards the cost of your care. Everyone is assessed based on their individual circumstances including their income, savings, other assets and any eligible expenses.
